Services: The Diabetes Prevention Program is part of the CDC-led National Diabetes Prevention Program, and is nationally supported by the Diabetes Prevention and Control Alliance and the Centers for Disease Control and Prevention • Helps those at risk of developing type 2 diabetes adopt and maintain a healthy lifestyle to reduce their chances of developing this disease • Trained Lifestyle coaches help participants learn about healthy eating, physical activity and other behavior changes • 16 core sessions held weekly plus 8 monthly maintenance sessions

• Financial assistance available to those who qualify – sliding scale fee based on ability to pay

• 18 years or older
• Overweight (BMI 25 or higher)
• At risk for developing type 2 diabetes
• Have been diagnosed with prediabetes by a healthcare provider
• No U.S. government issued ID needed
